If you are a dealer with a Dealer Resources username and password for this site, you can download the pro-forms directly from the dealer resources page. However, all pro-forms must be submitted to the companies by Hart-Montgomery Outdoor Sales with our signature to be processed. ANY PRO FORM SENT DIRECTLY TO THE COMPANIES WILL COME BACK TO US FOR APPROVAL AND WILL DELAY YOUR ORDER.

DEALER EMPLOYEES:

Ask the Owner or Manager to download the proform from their dealer resources account. Or you can submit the form on this page with shop employee chosen for pro type. Please get an Owner or Manager's signature on your proform when sent back to us.

Outfitters, Guides, and Industry Pros:

We believe that having our gear in the hands of professionals is a very important sales tool for both our companies and our dealers. We hope that you will understand that there is a process of verification for each pro deal that is requested. It is very important to us at Hart-Montgomery that the pro form process is not abused in any way. We also believe that for every pro form there should be knowledge of the dealer in your area that you will refer retail customers and clients to if there is any interest in a purchase.

To request a pro form fill in your information into the form. Choose the companies from which you would like to receive a pro form. Type the name of the dealer you will refer customers to. Upload a copy of your current guides license, letter of permission from an authorized dealer of the company, or other documentation of professional status. Choose your qualification type from the drop-down menu (Guide, Industry Pro, Store Employee). Give a brief description of who you are, who you work for, and where. Submit to us.

ANY PRO FORM SENT DIRECTLY TO THE COMPANIES WILL COME BACK TO US FOR APPROVAL AND WILL DELAY YOUR ORDER.

After we receive your request we will send the form or call with questions. Once you receive your form, you must submit the form back to our office. We will then sign it and get your stuff coming!
